Terms of Service
Last updated November 1, 2026
These terms are an agreement between you and FormzoAI for using our website, app, API and the forms we host. By creating an account or publishing a form you accept them. If you use FormzoAI for an organisation, you confirm you can accept them on its behalf.
1. The service
FormzoAI lets creators build forms, often with help from AI, and lets respondents answer them by chatting with an AI assistant or in a classic form. We also provide summaries, scores, insights, exports and integrations. We keep improving the service, so features may change; we won't remove a core paid feature in the middle of a billing period without telling you first.
2. Accounts and workspaces
- Accounts are free, for everyone. You need one to edit or publish forms; answering a form never needs one. Keep your sign-in method secure; you're responsible for activity in your account.
- Every account can keep a profile (contact details, links, work, studies, skills, projects) that fills forms when you choose to. Only add details that are true and that are yours to share. You check every filled answer before sending a form.
- Forms and responses belong to a workspace. The workspace owner controls it, including billing, members and deletion. Admins can manage members and billing; members can manage forms and responses.
- API keys and webhook secrets act on behalf of your workspace. Keep them secret and revoke any that leak.
3. Plans, trials and payment
- Building and previewing forms is free. Publishing a form and collecting responses needs a paid plan.
- Each workspace can start one 7-day free trial. It needs a card, includes Pro features and up to 50 responses, and turns into the paid plan you chose when it ends unless you cancel before then.
- Plans renew automatically each month or year until cancelled. Prices are shown on our pricing page, exclude taxes, and are charged in advance through Stripe. Except where the law says otherwise, payments aren't refundable.
- Each plan includes a number of submitted responses per month for the whole workspace. When you reach it, your forms stop accepting new responses until the next period or until you upgrade.
- If a payment fails, forms stay live for a 7-day grace period while we retry. After that, or when you cancel and the paid period ends, forms stop accepting responses. You keep read and export access for 90 days, then your workspace's forms and responses are deleted after an email warning.
- Event Passes are a one-time payment for one workspace, not a subscription, and nothing renews. A pass gives the features and the number of responses shown on the pricing page for 60 days, starting when you first publish a form after buying it. When the days or the responses run out, forms stop accepting responses and you keep read and export access for 90 days, as with an ended plan. Unused days or responses aren't refunded or carried over.
- If you buy as a consumer in the EU, you may withdraw from a purchase within 14 days without giving a reason. For an Event Pass, the service starts when you first publish a form; by publishing you ask us to start, and you then lose the right to withdraw from that pass. Email [email protected] to withdraw.
- We may change prices with at least 30 days' notice; changes apply from your next renewal.
4. Acceptable use
You agree not to use FormzoAI to:
- impersonate someone, or collect passwords, payment card numbers, or login details for other services (phishing);
- collect government ID numbers, health, biometric or other sensitive data unless you have a lawful basis and the respondent's informed consent;
- collect data from children without the consent the law requires;
- send spam, or harass, threaten, defraud or discriminate against anyone;
- break the law, or infringe anyone's rights, including intellectual property and privacy rights;
- attack, overload, scrape or reverse-engineer the service, get around limits or bot checks, or try to make the AI ignore its instructions to harm others;
- resell the service, or use it to build a competing product.
We may remove forms or suspend accounts that break these rules, without notice where needed to protect respondents or the service.
5. Your responsibilities to respondents
- You decide what your form asks and what you do with the answers, so you're responsible for having a lawful reason to collect them and for telling respondents how you'll use them.
- Every chat starts by telling respondents they're talking with an AI assistant and links to our Privacy Policy. Don't hide that or suggest a human is replying.
- Answer your respondents' requests to see, take or delete their data. Settings → Data requests finds everything one email sent your workspace, downloads it and deletes it.
- Your forms can link to your own privacy policy next to ours: link one you already have, or let us draft one under Settings → Privacy policy. A drafted policy is a starting point built from our template and your form's settings; it's your policy, so read it, check the details and add anything that doesn't fit. It isn't legal advice.
- Our Data Processing Agreement is part of these terms and applies whenever your forms collect personal data: you're the controller, we're your processor (Art. 28 GDPR). You can also accept it in Settings for a dated record.
6. AI output
AI writes form drafts, conversational wording, reactions and follow-ups, summaries, translations, insights and scores. It can be wrong, incomplete or inconsistent. Review a generated form before you publish it, and treat summaries and scores as help, not as the final word.
Don't make decisions with legal or similarly significant effects on someone, such as hiring, admission, credit or housing, based only on AI scores or summaries. A person should review the actual answers, and you're responsible for following the laws that apply to such decisions.
7. Your content
You own your forms and the responses you collect. You give us permission to host, copy, process and display them only as needed to run the service for you, including sending them to the providers listed in our Privacy Policy. We don't use your content to train AI models.
You confirm you have the rights to the content you upload, such as your logo and any imported forms.
8. Our property
FormzoAI, its software, design and brand belong to us. These terms don't give you any right to them beyond using the service. If you send us feedback, we may use it without owing you anything.
9. Other services
Imports from Google Forms, Typeform, Jotform and Tally, Google Sheets sync and your webhook endpoints depend on other companies' services and terms. We aren't responsible for them, and we only use the access tokens you give us to do what you asked.
10. Ending your account
You can cancel your plan any time from Billing; it stays active until the end of the paid period. We may suspend or end your access if you seriously or repeatedly break these terms, or if we stop offering the service, in which case we'll give you reasonable notice and time to export your data where we can.
11. Disclaimers
We work hard to keep FormzoAI available and accurate, but the service is provided “as is” and “as available”. To the extent the law allows, we disclaim implied warranties of merchantability, fitness for a particular purpose and non-infringement, and we don't promise the service will be uninterrupted or error-free.
12. Limitation of liability
To the extent the law allows, neither of us is liable for indirect, incidental, special or consequential damages, or for lost profits, revenue or data. Our total liability for any claims about the service is limited to the amount you paid us in the 12 months before the claim. These limits don't apply where the law doesn't allow them, such as for fraud or wilful misconduct.
13. Indemnity
If someone makes a claim against us because of your forms, the data you collect or your breach of these terms, you'll cover our reasonable costs of dealing with it.
14. Changes to these terms
We may update these terms. If a change matters, we'll email workspace owners at least 30 days before it takes effect. If you keep using FormzoAI after that, the new terms apply; if you don't agree, you can cancel.
15. Who we are and which law applies
FormzoAI is provided by Ansorjon Juraev, Eschersheimer Landstraße 42, 60322 Frankfurt am Main, Germany (Impressum). You can reach us at [email protected] or through the contact form.
German law applies, without the UN Convention on Contracts for the International Sale of Goods. If you're a consumer, you keep the protection of the mandatory laws of the country where you live. If you're a business, the courts of Frankfurt am Main have jurisdiction.
